Requirements

• • Proof of lawful presence in the United States (ITIN alone is not sufficient)
• • Social Security Number or SSA ineligibility letter
• • Proof of Alaska residency
• • Proof of identity and date of birth

Required Documents

• Valid passport with US visa or I-94 arrival record
• Employment Authorization Document (EAD) or I-797 approval notice
• SSN card or SSA denial letter
• Two proofs of Alaska residency (utility bill, bank statement)

Steps
